TITLE:                             Certified Nurse Aide (C.N.A.)

REPORTS TO:                 Charge Nurse & Assisted Living Director          




The AL/LV CNA is responsible for providing personal and non-personal care assistance to residents when there is a need and following each resident’s personal plan of care;  assists residents so that current living conditions can be maintained for as long as is safe and appropriate;  promotes independence with daily activities while giving assistance to each resident.




  1. Takes vital signs as requested.
  2. Assists with showers/baths, dressing, and other personal care of resident as indicated on plan of care.




  1. Assists residents with escorts to the dining room, hair salon, activities, etc. 
  2. Makes daily rounds to residents as required.
  3. Keeps Charge Nurse and Assisted Living Director informed of the status of each resident.
  4. Responds promptly to “Call for Aid” requests.
  5. Assists with meal service including table set-up, serving, tableside assistance, room trays, beverage service, dining room cleaning, etc.




  1. Performs light housekeeping tasks such as bed-making/changes, trash removal, personal laundry, etc. for each resident.


  1. Properly transports and delivers soiled laundry (linens) to designated collection points.
  2. Gives and receives accurate shift reports.
  3. Records accurate and timely resident progress notes, condition change notes, and other documentation as required and/or assigned.



  1. Completes all assigned tasks and items listed on shift checklist.

  1. Maintains safe, sanitary, and orderly conditions in all work areas.     
  2. Maintains all tools, equipment, and supplies in the proper condition.           
  3. Uses all safety equipment and follows safety procedures according to GCV policies.     
  4. Maintains an acceptable standard of personal hygiene and complies with GCV’s dress code.
  5. Maintains standards of confidentiality and resident dignity at all times.
  6. Assures that residents are treated with kindness and respect at all times.
  7. Maintains a high level of customer service for our residents and their guests at all times.
  8. Responsible for safety of residents in case of any type of emergency.
  9. Attends staff meetings, in-service training, and education sessions as required.            
  10. Performs other work duties and responsibilities as assigned by Team Leader and/or


Assisted Living Director.




  1. Current Oklahoma certification as Certified Nurse Aide (CNA).
  2. High School graduate, GED, or currently enrolled in high school.
  3. Ability to effectively communicate & follow instructions in English both orally and in writing.
  4. Ability to perform simple mathematics.
  5. Ability to understand measurements and conversions.
  6. Ability to document notes clearly.
  7. Minimum of 1 – 2 years care-giving experience preferred.
  8. Ability to plan and organize work, and to interpret instructions, specifications, and standards.
  9. Ability to work in close cooperation with residents, guests, visitors, families, coworkers, and management; requires courtesy, tact, and graciousness.




  1. Ability to follow and maintain basic infection control procedures.           
  2. Ability to recognize and respond to emergency situations.
  3. Experience in working with the elderly and/or health care settings preferred.



  1. Ability to stand/walk for 100% of the work day.
  2. Ability to work indoors and outdoors (as required).
  3. Ability to walk long distances at a rapid pace.
  4. Ability to assist in the bathing and dressing of residents with disabilities.
  5. Ability to independently push a wheelchair with a resident weighing up to 200 pounds.
  6. Ability to stand, turn, stoop, reach, bend, stretch, and balance for up to 95% of work day in order to assist residents.
  7. Ability to move equipment and perform other tasks using proper body mechanics.
  8. Ability to see, hear, and speak or use prosthetics that will enable these senses to function adequately to assure position requirements can be fully met.
  9. Ability to independently lift, balance, and carry up to 30 pounds.
  10. May be exposed to the elements (heat, cold, rain, snow).
  11. May be exposed to a variety of chemicals, dust, and offensive odors daily.
  12. May be exposed to communicable diseases and blood/body fluids that may contain bloodborne pathogens.
  13. Must be able to maintain mental & visual attention to diversified operations.
  14. Must be able to function efficiently despite distractions and interruptions.
